Abstract

Background: Benign Prostatic Hyperplasia (BPH) is a benign tumor that is commonly found on 1 in 3 men aged 50 years and more. Histological finding in BPH patient is an increase of stromal and epithelial cells in transitional zone. One main factor of this prostate growth is the change in sex hormone in the prostate. There are excessive testosterone to dihydrotestosterone converting by 5 alpha reductase enzyme. Prostate Specific Antigen (PSA) is a protein that are specific and produced by the prostate gland and can be used as a medical test for BPH patient to measure the disease morbidity. The use of dutasteride in BPH can slow down the process converting testosterone to dihydrotestosterone and prevent excessive prostate growth by inhibiting 5 alpha reductase enzyme. Tamsulosin is an alpha 1 receptor antagonist drug that can make smooth muscle tone of the prostate gland and bladder neck to relax. Therefore, the researchers want to investigate the effects dutasteride and tamsulosin drug combination on PSA levels in BPH patients in West Nusa Tenggara Provincial Hospital. Method: This study uses a comparative research design with an experimental approach and data collection methods by repeated measurements (pre-test-post test field trial). Data was collected at the Outpatient Installation of the Urological Surgery Polyclinic and the Medical Record Installation of the West Nusa Tenggara Provincial Hospital in the period of October 2019 to December 2019. Result: From 32 respondents who performed initial and final PSA examination, the highest average initial PSA level was found in the <4 ng/ml group and the average of the highest final PSA levels in the <4 ng/ml group. The average respondent comes from the age group of 61-69 years. Based on the hypothesis test using paired sample T test, the value of p = 0.035 (p < 0.05) showed that there was change in PSA levels in BPH patients who had been given a combination of dutasteride and tamsulosin drugs. Conclusion: There was change in PSA levels in BPH patients who had been treated using a combination of dutasteride and tamsulosin drug in West Nusa Tenggara Provincial Hospital.
